Identify a trustee to handle your estate on your passing
Distribute your property to your specified beneficiaries on your passing
If you have minor children or adult dependents, set up the mechanics for your trustee to manage your estate for your loved one’s benefit
Avoid public, time-consuming, and resource-consuming probate proceedings
An advance health care directive and an authorization to use and disclose protected health information to:
Identify an agent to make health care decisions for you in the event you cannot make them yourself
Give your agent direction regarding your wishes on certain end of life decisions
Give your agent access to the health information they need in order to make informed decisions
A durable power of attorney for financial matters to:
Identify an agent to make financial decisions for you in the event you cannot make them yourself
Transfer documents to transfer certain property into your revocable trust, such as:
One or more grant deeds for real property
A general assignment for non-titled personal property
A certification of trust to use with banks and the like when transferring accounts to your revocable trust
